Intended for California adolescents in foster care, this webpage contains information on extended foster care that allows adolescents to stay in foster care after they turn 18. The toolkit includes a brochure on extended foster care and information on coming back into foster care after leaving, housing options, supervised independent living placements, and the Transitional Independent Living Plan. In addition, education information is provided and addresses high school graduation alternatives, higher education options, tips for applying and paying for college, and the California Chafee Grant. Finally, a flyer is included on support services for transition-age youth.
