Funded through the Department of Health and Human Services, Administration for Children and Families, Children's Bureau, Grant #90CO1122, this National Quality Improvement Center for Adoption and Guardianship Support and Preservation (QIC-AG) tool presents a continuum assessment intended to assist partner sites in clarifying the existing services offered at each interval of the continuum, identifying gaps and strengths along the site’s continuum, informing the identification of evaluable interval assignment, and identifying areas for capacity building. The continuum assessment is composed of two separate but inter-connected elements. The first element gathers macro level organizational information on the site, including information on infrastructure, functioning, operations, and knowledge, ability, attitudes, critical reflection and evaluation, and resources. This information is organized by capacity domains that fall under process, outcomes, and cost. The second element gathers specific information about the programs/interventions that are offered at each of the intervals on the QIC-AG continuum framework: stage setting, preparation, focused, universal, selective, indicated, intensive, and maintenance.
