Funded through the Department of Health and Human Services, Administration for Children and Families, Children's Bureau, Grant #90CO1122, this National Quality Improvement Center for Adoption and Guardianship Support and Preservation (QIC-AG) tool provides a consent form for adoptive parents or guardians who participate in a stakeholders discussion about the quality and availability of services to families who have adopted or serve as guardians for the most vulnerable children. Information is provided on the need for informed consent for participation, the QIC-AG study on the quality and number of programs available to support families that are considering adoption or guardianship or who have adopted or taken guardianship of a child in their care, the length of the meeting, and the confidentiality of the information gathered.
