A proposal to implement a survey design that will provide national estimates of the incidence and severity of child abuse and neglect is presented. The survey model is designed to allow development of operational definitions of child abuse and neglect, derivation of associated methodologies capable of collecting data to determine the incidence and severity of child abuse and neglect, and collection of data to develop incidence rates. The survey plan involves a household survey of parents in 2 sites that would include a telephone component and a personal interview component. The model, which only covers the pretesting phase, also addresses the technical and ethical responsibilities of the contractor, tabulation and publication plans, and design scheduling. Copies of the household enumeration form, the telephone interview instrument, and the personal interview instrument are appended.
